Anonymous surveys

I'm curious to know if the survey's I email out are able to be kept anonymous, as in the person answering does not have to leave their name and email?

If the email field is not required the users do not have to enter an email address. 

You also do not have to add any fields to your form that will be used to collect personal info, like name, email, phone. 

However, the form will collect IP address of the user. 

How to Find the IP Addresses of Form Respondents

It is not possible to disable this option.
