Dynamic fields: how to create them?

I am creating an artist booking form. Upon choosing a number of performing members, the same no. of fields should be added in the form.

For ex. if 2 performing members are chosen, then 2 corresponding fields should be added in the form where the Artist can fill the Name and Role of the members who be performing. 

Please guide me if it's possible to do it or not.

So, I created 6 hidden text fields and depending on the number that you type on the "Number of band members" field, they show up (with the help of the conditionals).

If you type 2, then two fields show up. The maximum number is 6 but you can clone the form and change it according to your needs.

Can you please add 3 columns in every row, as shown below in the snippet and share the widget with me so that I can add it to my current form. Thank you!

I need the solution for max. 25 members

Now you have 3 fields per band member and it works for a maximum number of 25 members.

thanks agonhasani, for all the help and effort. I need to know if you can kindly give me the solution as shown below in the snippet. I would like Artist to fill the fields per band member in a single step only. I'm looking to upgrade my account today if a solution can be arrived for me.
