I have looked at your form and account details and noted that indeed the user submitted the form without filling the required fields.
I have escalated the issue to our Level 2 team for further analysis and review of the issue. We will update you on this thread once the root cause of the issue is determined.

Sorry for this inconvenience but I couldn't replicate the issue on my side although I can see some required fields are empty in that submission.
Did you change anything on the form lately? Maybe there might be a condition change?
