Need to create a similar form

I actually found out about Jotform from a form build by you guys already existing on carolinahealthspan.com, I want basically the exact form is there any way that we could make that happen?

I did check the website which you have mentioned but could not locate any embedded JotForm. I would suggest you to please share the webpage URL where you have embedded your form and we will help you build similar form.

Hello Justin - The easiest way to build a form similar to other forms is to clone it. However, due to possible copyright issues, it's best for you to get in touch with the form owner first and ask for permission if you want to clone his/her form, just to be safe.
