Hi there,

Can you please help?
Thanks!

I tried your form in Ipad Pro and in my case it was working fine. Are you using Safari browser or another one? And is your form not working in a specific case or specific element?
Please let us know so we can fix the issue. Thank you!

submitted later.
When we just use the online form, it has been problematic due to spotty
wi-fi in service locations.

Can you please tell me what application are you using to open the PDF file?
If you are not using Adobe Acrobat Reader, I recommend it. Here is a link to download it to your iPad: https://itunes.apple.com/us/app/adobe-acrobat-reader-view-create-convert-pdfs/id469337564?mt=8
I see that you have fields like Date, Time, Signature, which are validated. Even if the PDF is editable on your iPad, you need to be careful with the formatting.
