Our customers have been emailing us stating that they have filled out our form but that nothing is coming through to us.
I did a test on it, filled out the form, and I see that it isn't even on the list of submissions, our last submission was on July 27 when we have had countless customers fill it out.
Can you please let me know what is going on?

I tested your standalone form Return Request Form and it is working properly. I successfully submitted the form and my submission is show on the Submission page of your form.
Is your form maybe embedded? If so, please provide us the webpage URL, so that we can test the form there. If your issue is related to some other form, please provide us a link of that form.

I think that I found the reason of your issue. I checked your IP address and you have one more JotForm account. It is a guest_72075404558054 account and there you have one form that looks exactly the same as the form on your account. The name of the form is also the same. Here is the link of that form - http://www.jotform.us/form/72075446558161
I moved that form to your account MelieBianco.
