I figured out my others issues...but the HTML code from JF is not working

    I've posted a couple of inquiries for help - but I've more or less corrected those I believe. Right now I'm having difficulty imbedding the code into the website - its not matching what JF is confirming.


    Subjects Product Inquiry and Testimonial are not displaying correctly.

    Form works in JF but not in our website.  What am I missing?
