After jotform submission, three same cards are created in one of the board (Nancy Thompson). What is the solution

Thanks for reaching out to Jotform Support. I’m sorry you're having difficulties with this. I am unable to locate a form using your email address and IP address. Can you please share the form URL in question?

Thanks for getting back to us. I've cloned your form to test the integration and I was not able to replicate the issue. I've tested the form twice and there is only one card created per submission. Here are the submissions:
And here's what is shown on my Trello Board:
Are you using a different platform to integrate your form with Trello or you're just using our direct integration with Trello? Can you please try to remove the integration and integrate your form with Trello again? You can remove the integration by clicking the Remove Integration button.
Give it a try and let us know how it goes.
