how can i upload my pdf form?

i have a pdf form and have tried to upload it muliple times and keep getting error messages. i have tried all the links you guys have suggested 

As I understood your request. You have a form in pdf and you want to transform that to form in jotform? Unfortunately there is no widget you can use to do that. 

Please check form templates in our templates page  if you can't find a similar template you should build your form from scratch.
