I'm having a problem with the forms on your website. It keeps displaying the feedback information that is supposed to be sent to our customers before receiving the tracking number from us. Please advise. It used to look differently. I had a whole customer's data shown in there.

Can you please provide an example of what was before and what is showing now?  I cloned and tested your form and it was working fine in my end.
