Math Captcha widget on says "please run wizard"

The match captcha on my form says that there is an error when when I try to complete the form. Looking at the edit page, it says, "Please run wizard". How can the install wizard be run for the widget in Jotform?

I could not find any widget that has not been run. What is the form in question? Please note that you can simply delete it if you don't need it.
