    Hi Dance_Fusion_anthony,

Thanks for patiently waiting, I appreciate it. Just to confirm, do you want to generate a QR code for your attendees to scan and populate their information once the QR code is scanned? If so, you can use the Dynamic QR Code that allows you to generate unique QR codes on your form based on each form user's previous responses.  You can add a QR code on your form's autoresponder which will be sent to the form submitter by adding the code below on the source code:

    <img src="https://chart.googleapis.com/chart?chs=150x150&cht=qr&chl=https://www.jotform.com/submission/{id}&choe=UTF-8" alt="Your unique QR code" width="150" height="150" />

    Let me show you how:

    1. IForm Builder, in the orange navigation bar at the top of the page, click on Settings.
    2. In the menu on the left side of the page, click on Emails.
    3. Now, click on Add an Email.
    4. Next, select Autoresponder Email.

    5. Once added, copy and paste the code above at the very bottom of the source code page.

    6. Lastly, click the Ok button.

When the user receives the Autoresponder Email, they will receive a QR code. Then you can scan it and the submission data will show as a way for you to confirm.

Give it a try and let us know if you need more help.

    Thanks,

I would like to enter all of the information for the parents. Can I generate unique QR code from a Csv that I imported?

    Hi Dance_Fusion_anthony,

Thanks for reaching out to Jotform Support. Unfortunately, it's not possible to generate unique QR via CSV through Jotform, but you can consider trying other solution from third-party.

Let us know if you have other questions about your form.

    Hello,

How can this be done on the card layout?

    Hi Dance_Fusion_anthony,

Thanks for getting back to us. Unfortunately, the Dynamic QR code widget is only available in the classic form layout. I've coordinated with our back-end team to review this feature request. Rest assured, we'll circle back to this thread as soon as I receive a response from them.

We appreciate your patience and understanding while we’re looking into this.

    Hi Dance_Fusion_anthony,

Thanks for waiting patiently. I'm sorry for any confusion. Yes, you can follow the same steps explained by my colleague Shirized and achieve the functionality in a card layout form too. Add an autoresponder email and add the Google API in the source code of the email

Give it a try and let us know if you need any other help.
