How can I create a Form like the one I have attached in this submission.

    Greetings

    Such a nice form builder I got to Know. Many thanks for that. 

    I need to create form having table like the attached one. Please suggest me the best way to do it.

    Upon checking the for you shared with us — https://form.jotform.com/80034750118145, I see that you have used the Input Table fields to represent the table in your screenshot.

    I suggest that you also check the Configurable List widget and see if that will also meet your requirements.
