Error in cloning the form

tengo el siguiente formulario creado:

Cuando clono el formulario queda de la siguiente forma:

Como hago para que no cambie el formulario y quede exactamente igual al original?

I checked both forms and I can verify that they are using the same. It is possible that you were able to fix it by changing the Form Layout to Classic or All Questions in One Page.
