When I log in, I am getting a log in suspended error.

What do you mean "that was already there"?
Anyhow, as my colleague said, collecting credit card information through plain text is highly against our Terms of use. And we have a zero-tolerance policy regarding that rule.
The only way you can collect credit card information is to use one of our payment integrations: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm
Let us know if that works for you so we can remove the problematic fields and re-activate the account.
