Why is page 5 of my form not showing text?

Q3 - Why entire texts for page 5 are not visible ( Check the screenshot)? 

First off, your form is quite lengthy so it is loading slow on my browser. For lengthy forms, we don't really recommend the Continue Forms Later (Autofill). This sometimes crashes the browser. Please disable it and use this alternative solution instead https://www.jotform.com/help/97-How-to-Save-Forms-and-Continue-Later. 

With regards to the problem, you have multiple hidden form collapse on page 5. Some of them were hidden and shown by conditions so there's no need to actually hide them in the form builder. If you want to hide the form collapse, just set its button visibility to hidden.

So try this fix:

1. Show the form collapse

2. Use the Button Visibility option in the form collapse settings. If you want to hide it, set it Hidden.
