Cloned form lost original theme

I cloned NuLife Fitness Camp Questionnaire and the theme is gone and the available themes options are slim as if it reverted back to an older version.

That is because the cloned form is using a different layout (the new cards layout).

You can change the Form Layout by clicking SETTINGS, SHOW MORE OPTIONS, scroll down, change the Form Layout to the desired layout.

All Questions in One Page or Single Question in One Page.

To change the default Layout for your Account, go to your Account Settings and choose the desired Layout. This will not affect the forms created before.
