I created an account and immediately began creating a form, I was then later asked to login to my account again and it told me that my account was autosuspended and continues to say that. Please help me get back into my account, because I do not want to have to re create the whole form all over again.

Asking for login credentials is not allowed: https://www.jotform.com/help/8-Terms-of-Use \

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

If you want to have your account reactivated, we will need to delete the password field first.

Is there a widget that allows for that to be an option where as I am creating a site for a company that would like to allow user logins. Regardless I will remove the password field from the form once you allow me access back to my account.

We have removed that field, and reactivated your account. Unfortunately, there is no way to collect that type of information through a field or any widget.
