Form logo is still showing up even after deleting it

why is the logo keep popping on my form. I already deleted it 

I am indeed seeing the logo on your FORM PREVIEW even though it's not present on the FORM BUILDER. Looks like your image is still being loaded on top of the template for the form cover. I managed to remove it through the ADVANCED DESIGNER page.

To do that, go to your form's ADVANCED DESIGNER page > go to the DESIGN tab > expand the FORM COVER section > click the REMOVE button > then click the SAVE button on the left.
