How do I get my account un-suspended?

Your account has been suspended since you're asking for credit card information on your form 73017952330148.
Please note that asking for such information on plain text fields goes against our terms, for now I have removed the fields in order to reactivate your account, you will need to use one of our existing payment integrations if you need to collect payments (credit card numbers will not be stored on your account), here is also a link with all the information about payment forms: https://www.jotform.com/help/chapter-7-Payment+Forms
