How to create forms with different tabs?

    I have a website with three separate forms for visitors.  All three forms ask the same contact info, then ask different questions about the service they are requesting info or a quote.

    Is it possible with JotForm to create one form that asks the contact info just once and then can include the request questions for each of three different services?

    For example ... after completing the contact info section, can there be a tab or button to click that says "to receive a quote on marketing services, complete the following section."  If they skip that section, they see another tab or button that says "to receive a free kit on our insurance plans, complete the following section."

    Hope this makes sense.  Trying to streamline our website forms.

    Yes it is possible to achieve your requirement by creating a multi-page form. You can create a multi-page form where one page can have form fields asking for contact details and another two pages for different services.     Please note that you can also add "Form Tabs" widget to display pages of form in tabs as well.
