How can I create an appointment scheduling form?

I need to create an appointment booking form with a date specific calendar that has a slot blockers, so there is no double booking and that when place in my website I can control canceling and rescheduling of the appointments. 

This form could be use by a single user or have multiple. For example a hair salon that has 5 workers so to have a drop down an then schedule that shows for that person again so no double bookings.

I have tried to work with some of the widgets, apps and templates and are not able to get them to format to what I need. 

I have attached an example from a website that will be similar to what I am creating.

You can set how many slots you have for each day, any slot that is chosen will not be available for the next user.

For example, if you use date reservation widget, and when a date is chosen, the same date will not be available for the next user: 

Now, there is no way to preload the data from some previous user(s) and then choose from available, if that is what you had in mind?
