I created a form yesterday with the sole purpose of limiting who can fill out a form. Essentially, I password protected my form using conditions. My form was dissabled by jotform as a "login" form. I understand why this is not allowed but I am 99% sure this was done in error. My form is not a login form (I did name it that for lack of a better term though) It is simply to keep people from using the form when they aren't supposed to. Please give me some feedback and let me know that my account will not be suspended if I reactivate the form.
We have deleted the form to avoid suspension, otherwise I would send you a URL. I can redo them if you need to see that this is in fact not a login/phishing attempt.

We host millions of forms, and that is why mistakes may happen. I have whitelisted your "Changes Log In MASTER" form, so that your account will not be suspended in any case. Please feel free to re-activate that form.
