I have this issue with a couple of my forms. If you go to form https://form.jotform.com/80203798722156.
The question on this form asking if they are interested in taking the DREAM Score, if the answer is either of the first 2 (both different versions of YES), the Thank You page is supposed to redirect the user to a different page (www.dreamwellness.com/dreamscore) but that condition isn't working.

It seems that the URL in redirecting your Thank you page is incorrect.
It should be "www.dreamwellness.com/dreamscore" instead of Jotform.com/thankyou.html

The screenshot you have is actually different from what I see. I did in fact put the correct URL to go to dreamwellness.com/dreamscore but for some reason, it doesn't appear to be updating on your end. I tried it a few more times and now I am getting just a blank screen.

I believe that this is due to the form has Encryption enabled. Due to the encryption enabled the data that is supposed to trigger the condition is already encrypted thus it doesn't match anymore the condition.
I have cloned your form and turned off the Encrypt Form Data and it was able to redirect successfully.

Thank you for your help with this, but I am concerned that my problem won't be resolved because I need to use encryption. I took my original form and turned off the encryption. I then cloned the form, created the condition and it worked. I then turned encryption on for the new form and the condition stopped working. Will conditions such as these not work at all when using encrypted forms?

I am afraid that is the case. The conditions for the thank you page will not work if your form is encrypted.
That is because your form data is first encrypted and then submitted.
When the data gets encrypted, your condition has no effect anymore, as my colleague explained earlier.
I will escalate this to Level 2 and we will update you if this can be changed.
