    When the user fills out the form, it used to come to me in an email and the appearance was easy to read with spaces between the questions.  All I did was edit the form to replace an email address because of a change in personnel and now the email comes to me single spaced.

    Did I do something to cause this?  Thanks for your help, Linda

    This is the way it appears now:

    Hi,

    Have you checked your form notifications if it was set to mail out an HTML Codes? Maybe you have accidentally change the email notification to "Text Mode".

    Would it be okay for you if I ask you for the form URL where this issue happen?

    Thanks

    It is on a server for city of Jacksonville employees and cannot be viewed by the public. I placed the file on our public website server for you to see http://jaxpubliclibrary.org/website-posting.html .
    The form looks fine; however, the navigation guides look bad since their CSS file is on a different server.
    I tried the Text Mode and HTML mode and both sent the email the same.
    Thanks for your help, Linda

    I tried the Text Mode and the HTML Mode and they both sent the email with the same appearance.

    The file is on a server used only by staff of the city of Jacksonville, FL.  I placed it on our public website server for you to view; however, the navigation areas on the web page do not look right since they use a different CSS file on a different server.  The form looks the same.

    Thanks for your help, Linda

    Hi again,

    You have it in HTML format, however the HTML codes are displaced or removed. I would recommend to delete your existing form notification and recreate it again, please be sure not to alter the HTML codes because it may trigger an error or issue again.

    Here's what your email notification looks like at the moment (which cause you an issue);

    Hope this helps and please let us know for any concerns you may have, we will be happy to help you.

    Thanks

    Thank you so much Rodjit!  That fixed it.  I guess I was scared to delete the email notification.
