When uploading a file (for us, an image file) is it possible to upload text that is linked to the file? We want to upload images along with descriptions of the images. And then be able to see in the database entry a list of the images with their descriptions.
It would be OK to upload one image at a time -- that is, upload image 1 along with text description for that image, then upload image 2 along with text description for that image. Although ideally we would like to upload multiple images along with their text descriptions at once.
We now use another form builder (which as far as I can tell doesn't do the above). JotForm has a big advantage for us in that your forms can be saved for later filling out before submitting, which another form builder does not allow.
Our website is http://www.newbedfordopenstudios.org/
I did the form. Another person did the website.

We are sorry but it is only possible when you add upload field then a text box beside or below for them to add the description. It is also possible in Multiple Upload field but you have to remind users to upload text file (notepad) or word file that says it is for the image description and restrict other file format into that.
Single Upload with Text box: http://form.jotform.me/form/22780327245453?
Multiple Upload: http://form.jotform.me/form/22780306529455?
When you recieved a submissions, the descriptions are easy to find because they are in separate line, in Excel reports they are in separte columns.
I am hoping that this is good for you.
