There is no direct solution to make this happen on your form where the information to auto-fill the data would come from your own database. You will need to export your form's full HTML source into a certain page on your website, and then create an AJAX script that will pull the records from your database to your form. Unfortunately, we cannot provide a full solution for this since it goes out of our scope of Support but we can certainly point you in the right direction.
