My account is suspended but I have no clue why

    I need to talk about this immediately. I can't even login to rectify why it's suspended. I need to present this in front of my boss as a proof of concept in an hour

    Your account (camigirl4k3) is currently suspended. Our phishing detector tool automatically suspended your account. This form (Class Registration Signup Form) triggered the suspension.

    The form has a field that is asking for "Driver's License #". Collecting sensitive information is not allowed here in JotForm. Please check our terms of use.

    We need to delete the guilty field so that we can reactivate your account. If that is okay with you, please let us know. If you really need to collect this sensitive information, please do the following:

    1. An agreement stating the usage of sensitive information. You can use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.

    That's weird. I ....never put that there. 

    Please definitely delete that as it is not needed, it must have been on one of the pre defined forms that I loaded when I was testing. i have screenshots to prove that I never designated that field on the form that I made.

    Please deactivate and un-suspend my account.
