It was brought to my attention that several people had submitted the form http://form.jotform.com/form/10055023289 but I did not get an email notification. They were dated 09-30-12 to 10-04-12. I had previously gotten notifications from this form (dated 11-13-11 to 03-02-12) and nothing on the form has changed, including the email recipient information. I did try to send a Test Email and received nothing.
Fortunately, no real damage was done this time, but this has shaken my faith in your product a bit. Please help us figure this out.

Your email seems to be in our bounce list.
Also, I would suggest you to configure your email alerts' sender name and email to "JotForm" and "email@example.com", respectively.
If changing Sender name and email does not help, ask your hosting provider to add these IP addresses to their whitelist:
