Hi, I just had someone submit a form and didn't receive it. I haven't had any forms sent this month, so it wouldn't be that. Can you help?

Are you referring to the Maternity and Newborn Session Contract form?
I cloned this form and I did a test submission. After hitting the Submit button, it redirected me to the Thank You Page. Here's a screen capture:
The test was from the clone form. If you like, I can do a test submission on the actual form.
Is this form embedded on a webpage? If yes, then please share the URL so that we can test it. Also, I cleared your form cache so that your forms will be refreshed from our servers.

they said the submitted it today.
