I am unable to preview my form

    I am trying to preview a form that I'm working on and it's showing as currently unavailable.  The form is called Goal Intake Form.

    I checked your form and I was able to open it in Google Chrome and Mozilla Firefox. Here's a screenshot:

    If you are still having issues, please try to log out of your account and restart your browser. After that, please try it again and let us know if the issue persists.

    I have cleared your form cache as well so that your forms will be refreshed from our servers.
