Mailchimp Integration: Contacts not being added to list

Hi. I have integrated my form with MailChimp and when I test the form, none of the contact information is imported into Mailchimp. 

This happens with most of my forms I have integrated with MailChimp.

I cloned your form and tested the integration. I was able to reproduce the problem. It seems the address on your form is too short. You have specified to collect only the city name. But it is mapped to an address field in Mailchimp. And the address field in Mailchimp requires all address properties options to be visible.
