The above is a completed submission. The PDF is only showing some of the fields that are entered. I checked the settings and all fields are checked to display. This was working until recently.

It seems that you customized your PDF report and that because of that some fields are missing. To resolve this issue, please reset your PDF report. This can be done on the PDF Designer page.
