Please Verify My Form

    I used Google forms and they no longer support what I need. I am here on JOTFORM now because I need a solution ASAP. It is tax season and i need a reliable form to collect my client info.

    Please verify my form, to ensure it does not violate your policies. I have edited the settings to be encrypted & I have SSL on my website.

    I need help ASAP please!

    I am not going to share this with my clients until I get an approval from you guys.

    It is good that you sent this to us for a review since your account would be suspended for sure, and the reasons are:

    -You are collecting social security numbers,

    -You are collecting bank information,

    Both are against our Terms of use.

    So those fields have to be removed so you avoid the suspension of your account.

    We'll wait for your response.

    We will either remove thise 2 fields, or just find another company.

    TurboTax & HR Block

    They collect the same kind of information at their website so why can’t I?

     Big companies are killing us because of small issues like this.

     Please give me until the end of the day to find a solution please.
