Good evening, We are a tattoo shop that has clients fill out electronic consent forms through Jotform. We then have our Jotform account set up to also upload those forms into files in our Google drive. I am coming across several files that have multiple consent forms from multiple clients all in one file. For example on 3/15/24, we have a file in our Drive called '2024-03-15 Mario Arcila' and it has consent forms and ID images of 4 different clients. Can this be corrected? As it is time consuming to have to create new files for all the additional clients that are mistakenly put all into one file.

Hi Rebekah,
Thanks for reaching out to Jotform Support. I believe you are referring to this form. I've checked the Google Drive integration of this form and can see that it was set up to create a sub-folder for each submission. Please know that even if you have enabled this option, the integration will automatically sync the other submission on the existing sub-folder under the same name. You have selected to use the "Today's Date" field and the "Select an Artist" field as the folder name. So if there are multiple submissions on the same date for the same artist, they will be synced on the same sub-folder. To prevent this, I suggest adding another field that will make the sub-folder name unique. For example, adding the Name field.

to go after I get to the page I’ve attached?

Hi Rebekah,
Thanks for getting back to us. Adding another field to under the folder name of your Google Drive integration is easy, Let me show you how to do it:
1. In Form Builder, click on the Settings tab.
2. Click on the Integrations tab and select the Google Drive from the options.
3. Hover your mouse on the right side and click on the icon beside the three dots icon to Edit the integration.
4. Under the Folder Name, click on the Form Fields drop-down and select the Name field.
5. Once done, click on the Save button.
