I've consulted your FAQ and tried everything but I'm still not receiving the emails. Below is the url to my form.

I see you have 2 email notifications, but in the first one you have not specified a sender . I would recommend you to set up the sender as email@example.com on both.
Do that and let us know if you need further assistance
