    I am unable to get a suitable mobile view when accessing my form from a phone--I tried the mobile adjustment widget, but it did not work.  Suggestions?

    You seem to have added lots of Custom CSS which is why the form is not looking as it should on smaller screens. You have turned on the option to Shrink the fields and then using CSS you have overridden that.

    Try adding this Custom CSS to fix the width of the inputs:

    .form-line.form-line-column {

      width: 80%;

    }

    Let us know if you need further help.

    I have also change the Header of your form.

    To change it, please copy the source code below and paste it after clicking on the source code button in the text field. See the cast below on how to do that:

    <p><img style="display: block; margin-left: auto; margin-right: auto; width: 160px;" src="https://jufs.s3.amazonaws.com/14339481429128260" /></p>

    <h2 style="text-align: center; color: #555;">2018-2019 Monarch Band Audition Form</h2>
