Form not submitting

I created a form and embedded it. When I test filling in the form and submitting it, the submit button changes to "please wait", but then stays there, never actually submitting the form. Do I have a setting wrong that is causing this? Or some other issue?

I cloned your form, tested it and was able to submit the form.
