Account Suspended: Phishing

    I just had my account reinstated (thank you for the quick turn there) .... and Now I am blocked out again... Just trying to collect information for vendors and clients for our business and I'm evaluating the options and functionality as I go. Please follow up so I can continue working on this project. Will upgrading to a paid tier change anything here? Happy to do that, but I keep getting kicked from my account which is a little frustrating.

Upon checking, your account was suspended because your form Vendor/Client Info & W9 Form was automatically flagged as Phishing. Please have a quick view of Jotform's Terms of Use to know more about the restrictions and to avoid this happening again.

    If you believe that your account was suspended in error, we’ll need you to do the following:

    • Send us the URL where your form is used.
    • Send us a link to your website or proof of affiliation with a legitimate institution.
    • Provide any document that will support your claims.

    Send these things above to reviewers@jotform.com with Thread #13833321 as your subject, and we’ll let you know about the status of your account after we’ve done a thorough review of them.
