Error Message: Form Currently Unavailable - Followup

Thank you for your reply, but when I go into the Form Builder Page and the My Forms Page both, I cannot find the form I am looking for at all.

Additionally I am showing "Enable" as the current status under Settings

The document I am looking for is called "Store Level Transactions Submission Form".

Thanks for getting back to us. I understand that the issue is still not yet resolved that's why you reached out to us on another thread. However, we highly suggest you communicate with us on the other thread so you don't have to go back and forth as we might ask you the same questions when trying to troubleshoot the issue, which I know is not convenient for you.
