Please remove HIPPA requirement.

    Hello, I am not a covered 'Individuals, organizations, and agencies' that meet the definition of a covered entity under HIPAA. I am also unable to edit this form because of the block. Please remove the mandatory upgrade. Thank you.

    Hi Cheri,

    Thank you for reaching out to Jotform Support. May I know if you received an email stating that you may be a HIPAA-covered entity? If so, if you are not a covered entity under HIPAA, kindly respond to the email by stating that you are not. This way, your forms are not going to be disabled. We need to ensure that the users are not collecting sensitive health information without being covered.

    Also, you will need to access your Data Settings and select No I am not a HIPAA HIPAA-converted entity, so your forms will be reactivated. You will need to be logged in to your account. Let me show you how:

    1. In My Forms Dashboard, click on your Profile/Avatar in the upper right corner of the page and click on Settings.

    2. Select Data from the left panel menu.

    Now, if you have done the things above and the form is still not working, then you can check the status of the form if it is disabled. You can set the status of your form to Enabled in 3 easy steps. Let me show you how:

    1. Open your form in Form Builder.
    2. In the orange navigation bar at the top of the page, click on Settings.
    3. In the Form Settings window, set the Form Status to Enable. That's it, you're done.
