Issue with my form being flagged

In response to your email: Thanks for reaching out to Jotform Support. Your form named "Signature Verification Form" was flagged as a potential Fake Service Offer form, which caused the account to be suspended.

I only used the link on my IG page for my linktr.ee/taxspecialistee

Not sure why it was flagged. Or what else I need to provide. I used it as protections so clients can send me personal information like banking info and social security numbers verses using text. If the link has to be deleted that is okay. Can I create a new link?

1. The URL where your form is used.

2. Your website or proof of affiliation to a legitimate institution.

3. Any other additional document to support your proof to offer such service

Our relevant team will review your forms and information, and they'll inform you about the status of your account after a thorough review of your proofs.
