Issues with the formatting on my email notification

    The attached screenshot is output of report from form.  I've denoted my questions on image.

    1. I don't see any difference between the two address fields but there's a proper way to separate address field values and to strip off the address sub-labels. That is to use parameters in the field names. Here's a guide: http://www.jotform.com/help/92-How-to-Strip-Headers-from-Postal-Addresses-in-Email-Alerts

    Example:

    {input10:addr_line1}, {input10:addr_line2}, {input10:city}, {input10:state}, {input10:postal}

    2. The column names in your spreadsheet widget were generated through CSS hackaround. Unfortunately, that's only in the form level. It can't be carried over in the report. Since that is a spreadsheet widget, it will always use its default column names which are the numbers.

    3. Those are inside the table and has its own style. I'm afraid you can't customize it as of the moment.
