    I have added some fields to this form since I originally created it, and the form displays correctly on the website, but does not send me all of the correct information upon submission: http://www.dynamitesports.com/

    When I receive the email upon submission, the fields I added later do not show up.  The fields ommitted in the submission are:

    "Would you like us to find you on Facebook so you can receive recruiting tips in your newsfeed?"

    "Student-Athlete's Name"

    "Student-Athlete's Sport"

    "Athlete's Graduation Year"

    Hello wsimmons44.  To be able to get the correct information on your email alert, you will need to create a new one. 

    Or you can simply edit the existing email alert and then insert the new template variables (the one enclosed with { }) .  You can insert the new variable anywhere in your email alert by clicking the specific item on the Form Fields list on the left panel.

    Here is a sample layout of an email alert:
