My account was deactivated and the form i created was deactivated

I created a registration form and i logged in to check if it has been filled only to find out my account has been deactivated.

I also clicked the link and it said the link has been deactivated.

As I checked, your account has been suspended as your DREAMWORKS INTEGRATED LINK Form has been flagged as a Fake Service/Fake Job Offer. We need to delete it before we can reactivate your account. If, however, you are a legitimate institution and don't want us to delete the flagged form, then we need you to provide the following information.

• The URL where their form is used.
• Any document that will support your claims.
• Your website or proof of affiliation to legitimate institutions.

We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
