Hello,
I am interested in using Jotform to create online consent forms.
The issue I have is that I need two separate forms to link together. For example, first I need a school to provide consent and following that I need a parent to also provide consent. Ideally the school would complete their form and then a link would automatically be sent to the parent to complete their version. Once both forms are signed I would then be notified.
Is this possible with Jotform?

Hi Jack,
Thanks for patiently waiting. I understand that you want to link two separate forms to come through in one submission. Unfortunately, submissions from two different forms cannot be combined into one table. As a workaround, you can merge the two form submissions but the other submission form should be in another tab. Let me show you how:
- In the Jotform Tables, select the +Add Tab.
- In the Dialog Box, select the Tables and click Next.
- Select Connect to Forms, and click Next.
- Choose a Form to connect.
- Click Select All, and click Create.
