Question added to the form is not displaying on the form

I added an addtional question to the top of my form that shows up when I am in the "edit" mode of the form, but when I switch to "preview" it is not t

I see that there is a condition set on your form to show the field only when the set rules are passed. Please review the following condition on your form so that the field should be displaying normally on the form.
