Hello Customer HelpDesk,
I need help as my account is suspended. If you could also advice me the reason as to why it has been suspended so much the better.

Thanks for reaching out to Jotform Support. I'm sorry you're having difficulties logging in to your account. As I checked, your account has been suspended as your Alpha Micro Insurance Application Form has been flagged as a Fake Service. We need to delete it before we can reactivate your account. If, however, you are a legitimate institution and don't want us to delete the flagged form, then we need you to provide the following information.
- The URL where their form is used.
- Any document that will support your claims.
- Your website or proof of affiliation to legitimate institutions.
Please send these proofs to reviewers@jotform.com with Thread #13925691 as your subject. We will let you know about the status of your account after a thorough review of your proofs. We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
