Suspended Account: Copyrighted Logo

Hi, I wanted to edit the prices in the package I had created on JotForm my life coaching business.

Thanks for reaching out to Jotform Support.  I understand how frustrating this is for you, and I’ll do my best to help you. I checked your account and it is currently suspended because your form with the IDs 240157370222042 and 231405263614044 has a Copyrighted Logo added to it and is suspected as a Phishing form.

To re-activate your account, we will have to delete the offending forms. If you don't want these forms to be deleted, you need to provide us with the following proof of legitimacy:

1.  The URL where your form is used.

2.  Provide any document that will support your claims.

3.  Your website or proof of affiliation to legitimate institutions.

Send all of these to reviewers@jotform.com with "Thread #13945071" as the subject. One of our colleagues will get back to you about the status of your account.
