My account was autosupended while I was making a form. I'm not sure what happened. Can you fix? I just opened this account about 20 minutes ago.

You have been auto suspended due to the nature of the whole form. You are asking for personal information in the form including the driver license field which makes our system filter to tagged the form as a possible phishing form.
We allow this type of form, and I have already reactivated your account. But we suggest following the below recommendations:
